Doula Services Agreement & Outline
This agreement outlines the services, responsibilities, and expectations for both the doula and client. It ensures clarity, safety, and a supportive experience throughout pregnancy, birth, and postpartum.

1. Services Your Doula Will Provide
Emotional, physical, and informational support during labor and birth, at home and in the hospital, including guidance with breastfeeding immediately after birth.
Three prenatal meetings and four postpartum meetings, at your home or a location of your choice.
Assistance in creating a birth plan to guide your choices during labor and delivery.
On-call availability beginning three weeks before your estimated due date.
Unlimited phone and email consultations through the fourth postpartum meeting.
A backup doula to attend your labor and birth if your primary doula is unavailable. In rare cases, a doula may not be able to attend.
Collaboration with your healthcare team while respecting medical authority.

2. Services Your Doula Will Not Provide
Medical services: Your doula will not perform tasks such as blood pressure monitoring, fetal heart checks, vaginal exams, or other clinical procedures.
Medical decisions or advice: Your doula will not make medical decisions or provide advice regarding medical care. Your doula helps you understand information from your provider and supports informed decision-making.
Communicating with medical staff on your behalf: You or your partner speak with medical staff; your doula supports you and offers guidance.

3. Client Responsibilities
Attend all scheduled appointments with your doula.
Provide a designated contact person for birth notifications if the client cannot reach the doula directly.
Call your doula when you believe labor is starting; your doula will join at home if possible and provide guidance on when to go to the hospital.
Understand that late or missed notifications may affect birth coverage.

4. Attendance & Scheduling
The doula will attend labor and birth according to agreed coverage hours.
Both client and doula will communicate promptly about timing, location, or any unexpected changes.
Excessive lateness or missed appointments may require rescheduling and could impact care continuity.

5. Information Collection & Confidentiality
Personal, health, and birth-related information may be collected to provide safe, informed support.
All information is confidential and shared only with client consent or as legally required.
Documentation may include birth preferences, contact details, and notes about care provided.
6. Agreement & Understanding
By signing this agreement, both client and doula confirm understanding of services, responsibilities, and expectations. This ensures a respectful, supportive, and collaborative partnership throughout the birth and postpartum journey.
